Guide to completing the answer sheet
1. Declaration
At the top left of your answer sheet print your family name, followed by your other name(s). Sign your agreement to the conditions of the test below your printed name. Write today’s date (DD/MM/YY). Carefully copy down your passport or identification card number.
2. Information about candidate and sitting
Write the information in the boxes at the top of each section, then fill in the corresponding ovals below.
ISAT Candidate ID number - This is on the email
which confirmed your registration.
Test Centre number - This will be given to you
on the day of the test.
Test Booklet number - This will be printed in the top right-hand
corner of your Test Booklet.
Date of birth - Enter a leading zero in the case of single-digit
days and months. For example, 4th March 1979 would be 04 03 79.
Sex - Fill in the appropriate oval.
Test Form - The number of the Test Form is on the front cover of
your Test Booklet.
Country where final year of school
completed - There will be a
three-digit code for countries on the back cover of your test
booklet. View Country List
Latest IELTS score - Only include a score from IELTS. If you have
done any other English language test, please do not include the
score. If your score is a whole number (e.g. 6), please make sure
that you mark the answer sheet as 6.0.
Family name and other name(s) -
Make sure that you use the same names that you will include in
your university application. Use your legal names. Only include
an English version of your given name if there is room after your
legal given names have been entered. There are 17 columns for
your family name, and 17 columns for your other names. Don’t
be concerned if your names exceed the spaces provided.
3. Record of answers
The bottom section of the answer sheet has 100 numbered spaces where you record your answers to the test questions.
The answer sheets are scanned and scored by machine.
It is very important that you record your answers carefully. Alongside each question number, the letters A B C D E with a small oval around each letter represent the four or five alternative answers to each question. Mark your answer by completely filling in the oval corresponding to your chosen response. If you decide to change an answer, erase your first mark completely and mark the corresponding oval to your new choice.

When completing the answer sheet:
- mark your answer clearly by completely filling in the appropriate oval;
- use only a B, HB or 2B pencil;
- do not use ink or ball point pen;
- use a good quality eraser;
- be careful not to fold or tear your answer sheet;
- do not make marks outside the designated areas.
