Order Form

How do I order survey forms?

All orders for the Social-Emotional Well-Being Surveys should be made by filling in the form below. Fax the form back to ACER. ACER will despatch the surveys to your school. Return the completed forms to ACER where they will be scanned and a PDF report will be provided 10 to 20 days later. An invoice will also be generated at this time. Our payment terms are 30 days.

How many survey forms should I order?

If you are interested in making statements about your whole school, then, as many of the students should be polled as possible (esp. in schools with a population less than 200).

There are various permutations depending on your circumstances:

  • Cluster of Schools (more than 2,000 students, 50% of students should be surveyed)
  • Whole School (less than 200 students, ALL students should be surveyed)
  • Year Level (any size, ALL students should be surveyed)
  • "At Risk" Students (ALL students should be surveyed in a cohort of at least 30)

Example from a cohort of 2 primary schools and one high school:

  • Preschool: 50 students (across 2 primary schools)
  • Year 3: 50 students (across 2 primary schools)
  • Year 5: 50 students (across 2 primary schools)
  • Year 7: 50 students (across 2 primary schools)
  • Year 9: 50 students (at the high school)

Please note: To protect student anonymity, at least three students of each gender, at each year level, must complete a survey. Also teachers must complete a survey for at least 50% of the students, at each level surveyed, to receive comparative data.

To avoid being charged for unused surveys, you must return all surveys (unused and completed) to ACER. You will be charged for any unused surveys that are returned marked, torn, creased or are unfit for reuse.

Feedback via Reports

Feedback is provided at the level of Cluster*, Whole School, Year Level and Cohort (e.g., "At Risk" students); individual students cannot be tracked; year levels, whole school populations and clusters can be.

* Cluster level feedback will incur an additional charge.
